EEOC Select Task Force on the Study of Harassment in the Workplace

The information you provide will inform the work of the Select Task Force and will help the Equal Employment Opportunity Commission (EEOC) eliminate unlawful harassment. Your information (including any personally identifiable information you include) may be used in documents produced by the EEOC, and may also be shared: with a court, a party in litigation, a magistrate, an adjudicative body or administrative tribunal in the course of a proceeding; or with a congressional office in response to an inquiry from you to that office.

You are not required to share any identifying information other than your name and an email address at which we may contact you about your submission. Please do not provide your Social Security number.
